Case Management

Once a preliminary test has been administered indicating a health condition exists and the tested individual may benefit from care and services, introductions are made to a member of the MAO Social Services Team. The MAO team member will work with the individual to assess what is needed and assist or counsel the individual as to how best to access needed services – this is Case Management.

By its definition, case management is: “a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client’s health and human service needs. It is characterized by advocacy, communication, and resource management and promotes quality and cost-effective interventions and outcomes.” In short, creating an action plan to make sure you receive the best care, timely treatment and the supports necessary to improve health and aspects of life critical to quality of life.

Case management is an area of specialty practice within the health and human services professions. Its underlying premise is that everyone benefits when clients reach their optimum level of wellness, self-management, and functional capability. The stakeholders include the clients being served; their support systems; the health care delivery systems, including the providers of care; the employers; and the various payer sources.
